Our specially-trained diabetes nurse educators and registered dietitians work with you to map a plan for self-managing your diabetes. We offer small classes and individual sessions as needed. Our team approach will help you understand and learn to self-manage your diabetes in a relaxed and friendly environment.
During each session, the nurse educator will check your glucose levels and assist you with diabetes management, while the registered dietitian will create an eating plan that is personalized to your lifestyle. Your education and treatment may include:
- Carbohydrate counting and meal planning
- An exercise guide to help with blood glucose control and weight loss
- Monitoring your blood glucose level
- Learning how to recognize symptoms of hyperglycemia (high blood sugar) and hypoglycemia (low blood sugar) and how to prevent and treat each
- Identifying your blood glucose goals
- Reviewing precautions to prevent complications including: foot care, skin care, eye care, sick day guidelines, heart health and kidney protection
- Diabetes oral medicines: how they work, when to take them and what to be aware of
- Administering insulin or other injectable medications
