The Diabetes Care Center is an outpatient program that requires a doctor's referral to participate. Please fax doctors' orders to 804. 228.6754.

To schedule an appointment with the Diabetes Care Center, please call Diagnostic Scheduling at 804.327.8706. We ask that you arrive 30 minutes before your first scheduled appointment to register with Central Registration on the first floor of the hospital. Free valet parking is available in front of Levinson Heart Hospital or you may park in the deck.
